Components and Construction of the Curve

Interpretation and Cut-off Selection

Area Under the Curve (AUC)

AUC Value Interpretation Clinical Significance
AUC = 1.0 Perfect Test The test perfectly discriminates between disease outcomes with zero false positives or false negatives.
AUC > 0.8 to < 1.0 High Accuracy The test is highly capable of distinguishing between diseased and healthy patients.
AUC = 0.5 Non-discriminating The test performs just like chance and provides no clinical utility, represented by the diagonal line.
AUC < 0.5 Worse than Chance The test's predictions are systematically incorrect and worse than a random guess.
